Please call (630) 778-4774 between 9:00 a.m. and 4:00 p.m. CST, Monday through Friday. Please have your insurance information readily available. You should also verify the authorization requirements of your health insurance carrier. Once you have made an appointment:
If you have seen any other physicians for your pain problem or had any diagnostic tests, it is important that you forward these documents to CPM or bring them in to your initial consultation. This will assist the pain specialist in evaluating your problem. For your convenience, these can be faxed to our office at (630) 778-4725. If you need a release form, our office will be happy to assist you.

What to expect at your first visit
It is very important that we learn as much about you as possible, in order to effectively treat your pain. Your first visit will be scheduled for a one (1) hour slot. During that time you will be asked to complete a questionnaire detailing your pain and your medical history. A nurse will review that questionnaire with you. After you complete the questionnaire, you will be examined by the pain specialist physician. The physician will then map out a course of treatment.
